Obtaining a referral for a sleep study is as easy as visiting your family physician. Any licensed medical professional, whether it’s a physician, physician assistant or nurse practitioner, can refer you to the MEDIAS sleep center nearest you.
With the healthcare system as busy as it is in our fast-paced world, being prepared before your appointment is especially important. At MEDIAS, we have developed a Sleep Quiz (found in our Patient Resource Center) that you can print out and take with you to your appointment. It lists the most common symptoms associated with sleep disorders. Having this “medical checklist” will ensure that you are able to fully discuss your situation without the worry that you could be forgetting something important. If you do not have access to a printer, simply make a list of your symptoms to take with you.
As soon as we receive the physician’s order we will contact you to schedule the appointment. At that time we will obtain basic pre-registration information and schedule the test at a time and day convenient to you. We will then mail additional materials to you, including a sleep diary and medical history. This paperwork should be brought with you to your test – there is no need to mail them back to us.
