How do I refer a patient to MEDIAS?
Any physician, PA, or NP can refer a patient to one of our sleep centers. Just contact us at 800-633-4441 and request a faxed copy of our one-page order form. This form, which also serves as history and physical, is all we need to initiate the testing process. Complete the order form, making sure it is signed, and fax to 276-676-2170.
Our staff will contact the patient to schedule the study and obtain any additional information we may need. Once the study is scheduled with the patient, we will send confirmation with the date and time of the study via fax to your office. Should the patient change or cancel their appointment you will also be notified.
You will receive a detailed report of the study in approximately 7 days after the completion of the polysomnogram. During this time a registered polysomnography technician will review and score the study. Once this is complete, our board-certified physician will review the study and give a detailed interpretation of the study and a recommended course of action.
Should our physician recommend a follow-up study that includes PAP titration, we will automatically contact the patient and schedule the second study. Should the patient require a PAP device or any other home medical equipment, MEDIAS will send all reports and orders for the equipment to the patient’s home care company of choice. Patients are free to use any home care company they wish and to change companies at any time.
